Register Online with Archwood Medical Practice

We welcome new patients!

We offer a GP service to residents of Woodley, Romiley and Bredbury.

To register there are two forms to complete – the GMS 1 Form and a Medical Questionnaire. The information in the GSM 1 form helps quickly trace your medical records and request your records are sent to the practice. We also ask you to fill out a medical questionnaire because it can take time for us to receive your medical records.

Once we have received your forms one of our reception team will be in touch to verify your ID. Where possible we will do this via text link or video. ID can be: Passport, Driving Licence, Birth Certificate, National ID Document. Proof of address can be: Driving Licence, Utility Bill (dated within the last 3 months such as council tax, gas or electricity), Bank Statement (dated within the last 12 months), Tenancy agreement

To register children under the age of 16, one or more parent/legal guardian must be registered with the Practice.

If you are on regular medication then please send a list of this with your form if possible.

Once you email these forms it will take up to 7 days to register you on the system. You will receive a welcome email with your named GP and log-in details for Patient Access.
